These are starting points, not compliance forms. Adapt them. Where a template is used inside a live PMO, apply local ownership, version control and approval rules.

Figures and tables

All nine figures and all twenty‑three tables from the book, free, and on the same download page as the Companion Pack, from the one form below. These are the book’s own contents and you have already bought them; the email address is how I know who is using them.

Useful for putting a diagram in front of your own steering group, for printing Table 1.1 and working down it with a team, or for reading the wider tables across a page instead of down a phone. Nothing here replaces the book: a figure carries no argument on its own, and a table is an instrument the chapter around it tells you how to use.

Sizes, page count and the current edition are listed on the files page, which has the same three files with no form.

Reuse them in your own material with attribution. Figure 4.1 is an original diagram after a categorisation published by the CNSC, ONR and US NRC. The diagram is the author’s own; the underlying categorisation is the regulators’.

Case material. Bramwold, Stellar and Brackmere are composite cases. All organisations, contractors and individuals named in the case material are composites or inventions, and no reference to any actual organisation or person is intended or should be inferred.

If you have the book, it prints a direct link to these same files with no form on it. This page is for everyone else.

Searchable subject index and errata

Both are on the files page, and neither asks for anything.

  • The searchable subject index — the Kindle edition is reflowable, so it has no fixed page numbers and the index points to chapters instead. Type to filter.
  • Errata and updates — corrections found after publication, with the date and the affected chapter.

Spotted something wrong? Tell me and it will be checked, corrected in the next edition, and listed there.
